Vendor Registration

The ºÚÁϳԹÏÍø has employed a new initiative to enhance vendors’ ability to do business with ºÚÁϳԹÏÍø. New vendors seeking a business relationship with the ºÚÁϳԹÏÍø must:

Complete the following forms to submit with the Vendor Registration form:

W-9 Form(PDF)
E-Verify Certification in Compliance with Section 9 of AL Act 2011-535(PDF)
State of Alabama Disclosure Report(PDF)
Direct Deposit Form for Vendors(PDF)
